This page is divided into two sections, the top section contains the order form itself, the bottom section contains information you will need and can refer to when filling out the actual form. Refer to the bottom section as you fill out the form.
If, after reading through the questionnaire listed under the form below and referring to the website, you still have questions or are still confused about something, contact me and we'll work out what you need together that way.
You will not be billed for anything at this point. We will contact you after we receive your order and take it from there. You will not be charged for your urn until it is ready to fire. You may, however break up your payments if you like- paying half now and half upon delivery.
We accept all major credit cards through our secure, online billing service provided by QuickBooks. After we receive your order form, we will contact you regarding payment.

Design your own urn

1. Size of urn?
Size (for vase style urns*)
Dog: small (up to 30 lbs) approx. 6" tall x 5" wide
medium (40-60 lbs) approx. 8" tall x 6" wide
large (80-100+ lbs) approx 10" tall x 9" wide
Cat: one size, approx 6" tall x 5" wide
*Heights and widths will vary, so if you have a preference, please let me know.
2. Styles (prices given are for the basic versions of the style and include
name/dates and short inscription, they do not include extras such as
longer inscriptions, photo mounts, etc)
Cheers' Basic Custom Pet Urn - This urn starts with name & dates inscribed in decorative font on your choice of a heart, bone or plaque. From there the price can go up as customizations are added. These are: paw prints- $10-15, hanging tags-$15, biscuits-$20, additional inscriptions: Cheers' Favorites-$25, custom-$50, and photo mount-$50. For more choices at a reduced price, order a DIY urn, listed below.
Starting price: Cat and Small dog $75, Medium dog $80, and Large dog $85 (customizations add to this price)
Cheers' DIY (design it yourself) Pet Urn -This is where you get to put in your ideas to make this uniquely your urn. Designs or inscriptions can be either drawn or stamped into the pot or added as a raised design. Some examples that I've done in the past are: rainbows, carved squirrels, sun, moon & clouds, raised biscuits, attaching or incorporating bows & ribbons, dog biscuits, flowers, bugs, or paw prints stamped into pot. Stick to fairly simple designs and choose from our line of favorite inscriptions.
Cats and Dogs (all sizes) start at $150 and can go up, depending on the complexity of the request.
Cheers' Winged Kitty & Doggie Pet Urn - you may add; moon, stars and clouds or hearts
and balloons, etc.

Cheers'Cartoon & Custom Cartoon Urns These feature our trademark sculpted cartoon face, they can be glazed to match coloring (photo needed in some cases) or slightly altered to match breed for an extra charge

Cheers' Carved Portrait Pet Urn - A detailed illustration of your pet drawn on the urn while the
pot is still wet; a background scene of your choosing works well for this particular
style. Perhaps your dog loved the beach? A beach scene can be added, and so on.

(All sizes) $300 for one carved rendition
of pet, photos needed $500 for extended scene, include detailed description
of what you want, and send us photos we'll need, preferably in digital form.
3. Color
(glazes vary a little differently depending on clay used)
Washed Blue, Washed Green, Washed White,
(These glazes are nice light colors that break*(see
below) to a brownish earth
color)
Deep Blue, Deep Green, (these glazes are bold, rich, opaque colors)
Black, Rust (flushes from brown to rusty-red, this glaze can be
unpredictable!),
Opal-White (white with pinkish/purple overtones),
Light Tan, Dark Tan (this is just a clear glaze that lets the colors of the
glaze show through and I have dark, light and even black clays)
Stained (like "Max", we use an iron oxide stain which results
in a rustic, earthy look that lets the roughness of the clay come through (not
shiny like a glaze).
*Breaking glazes turn brown wherever the glaze is
thin, for example- on
rims and throwing rings or in places where the glaze is just applied thinly.
This makes for interesting effects, but is hard to control and the end result is
always a surprise.
Please be aware that some people find this effect to be "blotchy" while others consider it beautiful. It is impossible to control exactly how the glaze will break. If you wish for a solid appearance with few browned, breaking areas, but still like the color of the washed blue, green or white glazes, let us know and we will apply the glaze sufficiently thick to insure a solid appearance.
4. Picture Mount? Adds $50 , small picture no more than 3"x3", any bigger and the picture won't lie flush to pot because the pot is curved. The pot is altered a little to make a flat surface for the picture with decorative mounts. I can print up a picture sent via email if you like; my printer does a decent job. I laminate it so it lasts longer and to give it rigidity. I also may need to trim the picture to make it fit if this is an original photograph. It is also made so that you can easily replace the picture in the future as you'd like. (click here for example)
4a. Picture
Editing? Is the only photo or photos of your
pet a bit lacking? If you'd like, we can jazz it up for you, either
dramatically, adding special effects, or just touching up a less than perfect
photo. Currently, we're offering this service free of charge to our customers.
So, if you are interested, please feel free to inquire about this option.
5. Attaching collar or tags on vase urn? If yes, I don't really need you to
send me collars; although I have found that some specially sized or designed collars have required some simple
alterations in order to fit the pot better. If you are the slightest bit handy,
this won't be a task for you. Sometimes it simply requires a stitch in the back
so that the collar sits better, or a stitch so that the excess doesn't hang.
I have had to put in a new hole to make the collar fit a little tighter. This
was pretty simple too. If you feel better having me do this, then by all means,
send me the collar, I'm more than happy to do it. And definitely send in the
collar if it is a bit unusual, like extra small, or vise versa, so I know to
make the pot to fit. If you are interested in attaching only the tags, I can
make a small hole in the pot (either under the pet's image if there's room or
on the side) and I use a small wire to hang the tags off the pot. This will
require that you send the tags to me so I can attach them to the pot. PLEASE
SEND COLLAR, TAGS OR PHOTOS VIA CERTIFIED MAIL FOR YOUR SAFETY!!
6. Additional Inscription? (Other
than name and dates) - Additional words or short phrases can be inscribed into
hearts, clouds, dog bowls, doggie biscuits, etc. or they can be put directly
on the pot, please indicate which you prefer. Longer inscriptions may be framed
within an ornate border. Original
inscriptions should be kept short and add $50 to the price, our favorite
inscriptions are included in the price of our DIY
urns or can be added to other urns for an additional $25.Please note
the favorite inscriptions are
shown in their available font, you cannot choose your font.

Lid Sealing Options for vase urn: If you want the lid to be sealed permanently, I recommend
glue called e-6000, you can find it at most hardware stores. This glue is like
a silicone gel and really does the job. Super glue will work also and may be
easier to use. This is much thinner glue and may require that you apply thickly
for the pottery surface isn't totally flush. If you don't think you want a permanent
seal, I suggest using a silicone caulking that could be easily removed if needed. It forms a good seal and can be
easily cleaned away in the future. I make the lids heavy to fit like a plug. Special requests
can be made for a different style lid, or attachments to handle like a biscuit
or a rose bud, glass "eye", etc.

8. Payment
When your urn is completed and ready to fire you will be billed electronically should you choose to pay by credit card. DO NOT PAY US WITH PAY PAL!!. We accept all major credit cards. Online billing is done on a secure site run by Quickbooks. You may also leave your credit card info with me by phone or fax should you prefer. NEVER GIVE CREDIT CARD INFORMATION IN E-MAILS!!
Of course, we also accept checks and money
orders, and if you would like to break up the payment (half now/half when you receive
the urn) that is fine also. We do not require a deposit to begin work on your
urn, We only require that your urn be paid for upon completion and prior to
shipping.

We recommend if you are sending anything of importance (pictures, check, tags, collar, etc.), to certify it so we have to sign for it. Lately, there have been no problems, but we have had mail lost in the past or delivered to the wrong box, so use your own judgment in sending anything "too precious to lose"!!! We are not responsible for lost or stolen items sent to us.
Shipping is done through USPS Priority Mail and adds $15.
